NEW - NYS DOH Infectious Diseases Requisition

The Wadsworth Center at NYSDOH has updated the Infectious Disease Requisition (IDR) form to capture additional demographic patterns of COVID-19 infections, specifically information on both patient sex/gender identity and race/ethnicity. The revised form is attached here and can also be found under Healthcare Providers/Laboratory Forms and Requisitions. Please discard all previous versions and communicate the importance of completing this information to ordering providers. The use of older versions may cause testing delays.
